CONSUMER HEALTH DATA PRIVACY POLICY

1. Introduction This policy supplements the Less Time Fitness Privacy Policy and applies to "Consumer Health Data" as defined by the Washington My Health My Data Act (MHMDA). This policy explains how we collect, use, and share health data from consumers in Washington State.

2. Categories of Consumer Health Data We Collect We may collect the following categories of data for the purpose of medical screening and program eligibility:

  • Individual Health Conditions: Information regarding your weight history, BMI, and medical history.

  • Treatment History: Information about previous weight loss medications or surgeries.

  • Use of Prescribed Medication: Interest in or use of GLP-1 agonists (e.g., Semaglutide/Tirzepatide).

  • Measurements: Height, weight, and body composition data.

3. Sources of Data We collect this data directly from you when you voluntarily complete our eligibility quiz, intake forms, or communicate with our providers.

4. Why We Collect This Data We collect and use this data for the following limited purposes:

  • To determine your medical eligibility for our weight management program.

  • To facilitate the "Good Faith Exam" with our clinical partners (Qualiphy.me).

  • To provide the coaching and metabolic health services you have requested.

5. How We Share This Data We do not sell your Consumer Health Data. We may share your data with the following "Processors" to provide the services you requested:

  • Clinical Partners: Qualiphy.me (for medical intake and prescribing).

  • Service Providers: JotForm (for secure data collection) and Practice Better (for electronic medical records).

  • All processors are contractually bound to protect your data and limit its use to the requested services.

6. Your Rights Under the Washington MHMDA, you have the right to:

  • Confirm whether we are collecting or sharing your Consumer Health Data.

  • Access the specific data we hold about you.

  • Request deletion of your Consumer Health Data.

  • Withdraw your consent for the collection of this data.
